SKIER’S REQUESTED SETTINGS / SERVICES I authorize Olympic Valley Ski & Bike to perform such work that is necessary to properly mount, test and/or repair, including but not limited to, my skis, snowboard, bindings and boots. I have verified that the release setting numbers recorded on this form correspond with the numbers appearing in the visual indicator window of the ski bindings. As consideration for obtaining this setting and on the bindings or repair, mounting and slash or maintenance to boots, skis or snowboards, and to the fullest extent allowed by law, I agree to release from liability and never sue or make a claim against Olympic Valley Ski & Bike, the ski shop, its employees and agents, and the manufacturer and distributor, for any injury which results to me from use of these bindings, boots, skis, and Oregon snowboards or from participation in the sport of skiing I acknowledge that there may be increased risk of injury to me as a result of my own personal preferences for the binding setting, particularly in view of the inherent and other risks of the sport of skiing and expressly assume those risks.
